| For Calendar Year: | Enter the calendar year for the report.

| State Tax ID: | Enter your organization's State Tax ID number.

| Print with Amounts >= | Enter the amount that the IRS requires for the minimum for a 1099. Check with the IRS publication as the default may not be the current amount required.
