| Code: | Enter a unique code to identify the particular miscellaneous payroll deduction.

| Enter a description to identify the miscellaneous payroll deduction.
|
|
|
| Amount Type: | Enter an F if this amount is a fixed amount, or P if this amount is a percentage of the gross pay.
|
|
|
| Amount/Rate: | If the type is an F enter the amount to be deducted each pay period.
|
| If the type is a P enter the percentage as a rate (i.e. if 12.3% enter 0.1230).
|
|
|
| G/L Liability Account: | Enter the General Ledger liability account to be used to store the amount withheld before it is paid.
|
|
|
| A/P Vendor: | Enter the Accounts Payable Vendor code for the Vendor to be used when paying this deduction.
|
|
|
| Employer's Portion
|
|
|
| Amount Type: | Enter an N if there is no employer's portion, an F if this amount is a fixed amount, or P if this amount is a percentage of the gross pay.
|
|
|
| Amount/Rate: | If the type is an F enter the amount to be deducted each pay period.
|
| If the type is a P enter the percentage as a rate (i.e. if 12.3% enter 0.1230).

|
| G/L Expense Account: | Enter the General Ledger expense account to be used to record the expense of the employer's portion of this deduction.
|
|
|
|
|
| W-2 Box 12 Indicator: | Enter the Code to be used on the employee's W-2 form, box 12.
|
|
|
| Skip Extra Pay Periods? | Indicate if this deduction will be skip on extra pay periods.
|
|
|
| If a person is paid biweekly, there are normally two pay periods each month. Four times a year there are three pay periods in a month. If this field is checked, the deduction won't be taken on these four extra pay periods.

|
|
| Taxable ... | Indicate whether or not this deduction is taxable for the indicated tax.
|
|
|
| If these fields are NOT checked then this deduction will be made from the gross amount paid before the specified tax is calculated.
|
|
|
| For example, Employer contributions to a qualifying pension plan is exempt from withholding, Social Security tax, and Medicare are not taxable.
